Jema Support Services is dedicated to providing high care to those who live with Type 1 and Type 2 Diabetes, which is why we have an on site, credentialed Diabetic Educator/Registered Nurse who can assist you with all of your education and management needs.
Diabetes Services Include:
- Comprehensive assessment which includes 3 home visits a day (for 5 days) to record Blood Sugar Levels and record food intake to help us understand your current diabetes management.
- 24/7 Access via phone to our credentialed Diabetic Educator during our assessment process.
- Custom Management Plan to help you manage your diabetes.
- Liaising with your GP/specialist (or our in-house GP) for possible treatments options and/or medication changes
- RN/EN Services available for medication training/administering
