When you become a part of the Jema Support Service family, you are granted your own specialized support co-ordinator. your support co-ordinator is trained to offer you the best possible suggestions, co -ordination of support and solutions you could possibly receive. This service is time-limited, and focuses on addressing barriers and reducing complexity in the support environment, while assisting family to build capacity and resilience and connect with other support. It may also involve developing an intervention plan that will be put in place by disability support workers.
Your support co-ordinator will be in charge of...
- Assist the participant at the specialist level to implement their NDIS plan by maximizing choice and control and engaging high quality service providers.
- Contribute reporting and participate in review processes for the NDIS participants as requested or required.
- Manage and coordinate any complex care team requirements for the participant where they have specialist support coordination.
- Maintain accurate records, relevant reporting and communication in relation to the participant and their care team.
- Provide information to the participant and their care team in the language or mode of their choice.
- Ensure that conflict of interest in any implementation of supports or management of the care team and participant need is clearly documented, explained and managed with the participant, their support network and the organization.
- Ensure that the optimal mainstream and community supports are utilized by the participant wherever possible and ensure the participant receives value for money services when NDIS funds are spent to achieve the objectives of the participants plan.
- Find community services and resources .
- Identify and evaluate potential support options
- Make referrals, and negotiate and contract with support providers.
- Analyse the need for secondary support.
- Write, cost and implement support plans.
- Coordinate support arrangements.
- Conduct regular reviews to ensure effective service provision.
- Advocate with you or on your behalf if your rights to services or opportunities are being compromised.
- Provide key information about government benefits.
- Assist with advanced care planning, including obtaining guardianship and/or administration orders if necessary.
- Participate in multidisciplinary teams and meetings with external agencies, e.g. child protection, mental health or parole and correction agencies.
